Door locking issues

I was dropping off my kid the other day. She attempted to open the passenger door before I had the car in park . It ( front) passenger wouldn't open. She climbed over seat and got out the passenger door. I thought this strange and thought little more of it. I was in my car when a friend went to open the door ( drivers) while I was parking - I'm near certain it was in drive and when I stopped the car I couldn't open the door from inside. I cycled the ignita few times and nothing would work. I opened the window and stopped engine- put key into lock and tried that - no luck. I ended up climbing over seat and out through rear door. I went to get. Ack in and was locked out- also the fob has been broken for along while. I ended up breaking into my own car via the passenger rear quarter window. I started up car and drove away. It hasn't done this since- now I'm looking for a new window. Has this happened to anyone else- is this a symptom of more woes with locks

what year is your car?
if the car is in drive with a running engine your front seat passenger/child will not be able to open the door.
when you couldnt open the door, the engine was off, car in park, and you tried the lock switch on the arm rest?
when you climbed over the seat and got out the back, you didnt take the car key with you?
