98 Suburban engine squeal

Trying to diagnose & resolve a mysterious engine squeal. 1998 Chevy Suburban 4x4 ½ ton. 220,000 miles, original 350 engine & automatic transmission. Starts, runs, and drives quietly—only begins to squeal after it has warmed up for 10 minutes or so. If car is turned off and restarted while engine is still warm, squeal resumes immediately. Squeals with transmission in “park” or in any gear. Squeal continues even when serpentine belt is removed. Squeal seems to come from rear of engine, is loud, and sounds like metal-on-metal.
 
did you try removing the cover over the torque convertor?
 
You sure the starter bendix isn't staying engaged into the flywheel?
