1993 Chevy Silverado overheating?

My truck is at normal temp when I am driving and when I idle it goes all the way to the red. When I stopped and popped the hood I touched the radiator cap and it wasn't hot at all. No steam anywhere or anything. When it gets all the way to the red when I idle, as soon as I take off it goes all the way back to normal. Help me !
 
Check to see if the cooling fan is working properly
 
bad water pump...assuming you have coolant/water full....pump,,,pumps faster at higher speed.....also could be thermostat,,...everything is 15 yrs old...may be combo problem...clogged radiator...bad clutch fan/or relay if electric...old hoses can go flat at higher pressure....
 
If it has a clutch fan, the clutch unit needs to be replaced. Sounds like a lack of air flow. See if the fan spins freely.
